Start with the requesting agency
Your employer, licensing body or other requesting organization supplies the request instructions. If the agency section is missing or you do not know which service you need, contact that organization before traveling.
Do not copy another person’s form or choose codes from an unrelated online example. A correctly captured fingerprint sent with the wrong agency details can still fail to solve your request.
Confirm identification before the visit
Bring valid, unexpired photo identification. California DOJ does not accept expired identification for this process. Its fingerprint roller handbook lists a California driver’s license, DMV identification card and an out-of-state driver’s license as primary forms.
If you do not have one of those, call the operator before traveling. Alternative identification can require supporting documents; do not assume any photo card or a photograph of an ID will be sufficient.
Tell the operator which current photo identification you intend to bring and ask whether it meets its requirements. If a name differs between your ID and request, ask the requesting agency how to resolve it before submission.
Requirements for a separate program enrollment appointment can be different. For example, the county’s IHSS enrollment appointment has its own ID and Social Security card requirements; that does not turn this page into a universal document list for every program.
Before leaving the counter
Check the information entered by the operator. Keep your applicant copy, ATI transaction identifier and receipt together. Ask the agency whether it needs a copy and how it wants to receive it.
The ATI identifies your submission. It is not a certificate that your background check is complete.

Can I arrive without a request form?
For an agency-requested California Live Scan, get the request from that agency first. Call the location if you need help identifying the service, rather than assuming it can supply your agency information.
Does this website accept my documents?
No. Do not send us IDs, Social Security numbers, fingerprints or background-check records. Give sensitive information only through the operator’s or agency’s authorized process.
Can I print this checklist?
Yes. Use your browser’s Print command. The checklist works without an account or client-side scripts.
