Live Scan request form and ORI: avoid the wrong submission

Understand who supplies your Live Scan form, why agency codes matter, and what to check before your fingerprints are submitted.

Use the form and instructions from the organization requesting your check. The agency information routes the response; a blank form alone does not tell you which request to make.

Who completes the agency information?

Ask the requesting organization for its prepared request. Check its name, ORI and required levels of service against its instructions. If any of these are missing, ask the agency to clarify them. Do not invent a code or reuse a previous employer’s details.

Review at two points

  1. Before traveling: make sure you have the correct request and know which identification and payment to bring.
  2. At the counter: compare what the operator enters with the information on the form before transmission.

This second check matters even when your paper form is correct: information still has to be entered into the Live Scan system.

If a request was already submitted incorrectly

Contact the requesting agency and the original operator. Explain the field that appears wrong and keep your original transaction information available. California DOJ warns that most errors cannot simply be corrected after submission; the next step depends on what went wrong.

Do not submit a second paid request based only on a guess. Get the responsible agency’s instructions and ask about any charges first.

Applying to another organization

Ask the new organization whether it requires a new submission. Do not assume an old receipt or status screen will satisfy a different employer or license. A fingerprinting appointment is linked to a particular request, not an all-purpose clearance document.

Can this guide give me the correct ORI?

No. Get the ORI and application instructions from the organization requesting the check. A generic code could send information to the wrong destination.

Is the ATI the same as the ORI?

No. The ATI identifies a fingerprint transaction; the ORI identifies an agency. Keep the transaction details from your completed visit.

Should I use a blank form found online?

Use the specific request your agency supplies. If it directs you to a blank form, ask it to confirm the agency fields and required service before submitting.
