CTC form or district instructions?
CTC uses its 41-LS form. Clearance obtained through another employer does not exempt a first-time CTC applicant. Keep your legal name consistent across the request and credential application. Open the current CTC instructions and form.
Where to arrange the local visit
The county credential office recommends Live Scan Fresno on Bullard and All American Live Scan in Clovis for CTC requests.
For participating school districts, the employer schedules FCSS consortium appointments. Do not substitute a public walk-in for that arrangement. FCSS publishes weekday fingerprinting appointments from 8 am to 3:30 pm and a $70 CTC fingerprint charge; confirm which route applies before paying.
Keep the two requests separate
Ask the district: “Is this appointment for employment, CTC, or two separate submissions?” Keep each applicant copy and ATI receipt identified by purpose. Follow the status-check steps for the relevant transaction.
